Support built around the individual

The approach should match the participant’s pace. Progress is most meaningful when the participant is actively involved in choosing what to work on and how support is delivered.

Transport and travel support

Bonte Care can provide transport and travel assistance connected to a participant’s support needs and plan. This may help with appointments, community activities or other agreed destinations.

The team should confirm the purpose of each trip, the type of assistance required and any relevant pricing before the service is provided.

Community participation

Community participation support can help participants connect with people, places and activities that matter to them. This may include social, recreational or community-based activities selected by the participant.

Support should promote genuine inclusion and choice. The participant remains at the centre of decisions about where they go, what they do and who supports them.

Personal care

Personal care support may assist with agreed daily personal activities. It should always be delivered with privacy, dignity, consent and respect for the participant’s routines and communication preferences.

The level of assistance should be matched carefully to the person’s needs, while supporting independence wherever possible.

Household tasks and shared living

Support with household tasks may help participants take part in maintaining their home and everyday routines. Shared-living support should be organised around the participant’s individual needs, rights, choices and living arrangements.

Clear boundaries and responsibilities are important, especially where several people or providers are involved.

Plan management

Plan management can help participants with provider invoices and financial record-keeping for plan-managed funding. A plan manager should provide clear information, help participants monitor their budgets and support informed decisions without taking away choice and control.

Participants remain entitled to ask questions, review their spending and choose the providers that suit them.
